Instructions For Domestic FedEx Shipping:

As part of our service, we pay for shipping costs to and from our lab. We will provide you with pre-printed FedEx Priority Overnight shipping labels with our account number on them. In order to maintain this valuable service, we ask that you verify the following information and adhere to these guidelines when you ship packages to us:

• Box 1 – Fill in your name, address, and account number, if applicable
• Box 7 – Verify “Bill Recipient” is marked with Account Number 1386-0797-6
• Keep the 1st copy for your records
• Call FedEx for a pick-up at 1-800-GO-FEDEX (1-800-463-3339)

PLEASE NOTE:

• If your office is running low on pre-printed air bills, please let us know and we will send more right away.
• It is not necessary to return empty crown boxes as they cannot be re-used.
